All intake and consent forms must be submitted at least 12 hours before your appointment.
Please check your text and email for important information about arriving at your appointment, including our address.
We are only open by appointment and do not take walk-ins.
Cancelation Policy
If you need to cancel your appointment, give us at least 24 hours’ notice, and there will be no fees. If you need to cancel less than 24 hours before your appointment time, you will be charged 50% of the cost of your scheduled service.
